Before you wear a respirator for work, you must complete a questionnaire that asks you questions about your medical history. Your responses are strictly confidential and are not shared with your employer. They are submitted via a secure connection to a contracted medical team.

You will enter your personal identifying information, and begin the medical questionnaire. Once you are done and have answered all the questions, click the green button that says "Submit MEQ Record." If you have completed the questionnaire correctly, you will then be forwarded to another page with your results. 

If you click "Submit MEQ record" but are not taken to another page, it means you missed answering one or more questions. Go back and review your responses, then try to submit the questionnaire again.

Antes de usar un respirador para el trabajo, debe completar un cuestionario que le hace preguntas sobre su historial médico. Sus respuestas son estrictamente confidenciales y no se comparten con su empleador. Se envían a través de una conexión segura a un médico contratado.

Se entra en su información de identificación personal, y comenzará la evaluación médica. Una vez que haya terminado y ha contestado todas las preguntas, haga clic en el botón verde que dice "Enviar Formulario". Si ha completado el cuestionario correctamente, será reenviado a otra página con sus resultados.

Si hace clic en "Enviar Formulario" pero no se le lleva a otra página, significa que no respondió a una o más preguntas. Regrese y revise sus respuestas, luego intente enviar el cuestionario nuevamente.
